I think you should start by first starting to plan in detail of how much syll. you have to cover in the days which are still left. Stay calm and don't over plan everything. Don't set huge goals in a single day. Take breaks in between. Don't study for long hours. Prefer quality over quantity. As you mentioned you are having trouble concentrating , one thing you can do is eliminate all types of Online and offline distraction for a temporary amount of time. Like don't use smartphones while self studying only use them for lectures or taking notes and stuff. Make a plan and stick to it. If you have a study room use it only for studying , give good positive motivational quotes in your Mobile wallpaper which can help you when you get distracted. Follow these things surely it will help you . I myself used to face concentration issues, so I started with 10 mins of meditation and yoga everyday. Trust me it really helps

Is something else bothering you? If yes, then keep it aside for a while. Start your morning with yoga to relax your mind. Prepare a schedule and strictly stick to it. It's only a few days of hard work until your exams are over! And keep your phone aside while studying. Take short breaks to refresh your mind in between.
