I think you should try daily agenda setting and restricted breaks after every 45 mins.

Take it from me, I was a gold medalist in engineering. Before you start studying, keep your phone away, in silent mode. Do not check for msgs/calls. Go to a quiet room and study as per a defined schedule. Devote time to each chapter/topic. At the end of the day, revise everything. The satisfaction you'll get after one day of dedicated study will serve as a motivation for day 2. If you have strong will power and won't be distracted by phone/TV/laptop, this will work wonders. Take short breaks in between, but don't waste time going out or talking unnecessarily even at home. You can do all of that after your mission for the day is completed. All the best!

Great! I am preparing for my neet pg exam. My exam dates are uncertain and I feel 19 subjects as very heavy, so am struggling out here, losing confidence in between, loosing flow in between, really worried of it!

Nothing to worry, do as I said. You need to be confident. Prepare a schedule and strictly stick to it. Divide time for each subject and with thorough revision, you're good to go. Keep your phone away, though!
